Skills

• Bilingual (English/Spanish) skills are a plus!
• Basic Life Support (BLS) certification.
• Proficiency with medical instruments and equipment.
• Excellent communication skills (written and verbal).
• Knowledge of computer-based data management programs and information systems.

Responsibilities

• Conduct assessments, including comprehensive annual wellness visits, both in patients’ homes and virtually.
• Counsel and educate patients and families about available benefits and programs.
• Document patient information, including chief complaint, diagnoses, history, review of systems, examinations, medications, allergies, assessments, and plans.
• Coordinate care with primary care providers, specialists, and ancillary services.
• Complete all documentation promptly and accurately.
• Maintain high-quality care standards.
• Provide evidence-based, timely care that reduces avoidable hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
• Prescribe medications, order tests, and collaborate with patients’ physicians.
• Work effectively to improve patient quality outcomes.
• Educate patients about preventive monitoring and collaborate with physician practices to schedule diagnostic testing.
• Assist patients with accessing educational resources.
• Participate in integrated care team meetings.
• Other duties as assigned.

Qualifications

• Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
• Board certification (NP: ANCC/AANP; PA: NCCPA).
• Current and unrestricted DEA certificate.
• 2+ years of direct patient care experience.
• Managed Care/IPA/Health Plan experience is a plus.
• Experience conducting annual wellness visits or similar comprehensive visits (virtual or in-home) is a plus.

Education Requirements

• Graduate degree in Nursing or Physician Assistant studies.
