Skills
Bilingual (English/Spanish) skills are a plus!
Basic Life Support (BLS) certification.
Proficiency with medical instruments and equipment.
Excellent communication skills (written and verbal).
Knowledge of computer-based data management programs and information systems.
Responsibilities
Conduct assessments, including comprehensive annual wellness visits, both in patients’ homes and virtually.
Counsel and educate patients and families about available benefits and programs.
Document patient information, including chief complaint, diagnoses, history, review of systems, examinations, medications, allergies, assessments, and plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation promptly and accurately.
Maintain high-quality care standards.
Provide evidence-based, timely care that reduces avoidable hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
Prescribe medications, order tests, and collaborate with patients’ physicians.
Work effectively to improve patient quality outcomes.
Educate patients about preventive monitoring and collaborate with physician practices to schedule diagnostic testing.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Other duties as assigned.
Qualifications
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
2+ years of direct patient care experience.
Managed Care/IPA/Health Plan experience is a plus.
Experience conducting annual wellness visits or similar comprehensive visits (virtual or in-home) is a plus.
Education Requirements
Graduate degree in Nursing or Physician Assistant studies.

Minimum 2 years of direct patient care experience.
