Skills

  • Bilingual (English/Spanish) skills are a plus!
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Proficiency with medical instruments and equipment
  • Excellent communication skills (written and verbal)
  • Knowledge of computer-based data management programs
  • Understanding of federal and state regulations (HIPAA and OSHA)
  • Ability to work independently and practice autonomously

Responsibilities

  • Conduct assessments, including comprehensive annual wellness exams, both in-home and virtually
  • Counsel and educate patients and families about available benefits and programs
  • Document patient information, including chief complaint, diagnoses, history, and treatment plans
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Complete all documentation promptly and maintain high-quality care standards
  • Deliver evidence-based, timely care that reduces avoidable hospitalizations
  • Facilitate closing gaps in care by educating patients about preventive monitoring
  • Participate in integrated care team meetings
  • Perform other duties as assigned

Qualifications

  • 2+ years of direct patient care experience
  • Experience in Managed Care/IPA/Health Plan is a plus
  • Experience conducting annual wellness visits or similar comprehensive visits is a plus

Education Requirements

  • Nurse Practitioner or Physician Assistant program completion

Education Requirements Credential Category

  • Licensure in the state of practice

Experience Requirements

  • 2+ years of direct patient care experience
