EAC Network is seeking a bilingual Case Manager (English/Spanish) for its Nassau Adult Guardianship program in Nassau County. This role is crucial in managing personal needs and property management for incapacitated adults, focusing particularly on Spanish-speaking clients. The position operates Monday to Friday from 9:00 AM to 5:00 PM, with flexibility for evenings and weekends.

Skills

  • Proficient in office and computer applications (Microsoft Office, spreadsheets, email, etc.)
  • Bilingual (English/Spanish) required
  • Excellent interpersonal and communication skills, both oral and written
  • Ability to motivate and advocate for clients

Responsibilities

  • Contact relevant parties within 72 business hours to initiate the fact-finding process and service plan development
  • Provide case management support through home visits, telephone reassurance, and advocacy
  • Transport and escort clients to appointments as necessary
  • Participate in case conferencing for reviews and decision-making
  • Maintain up-to-date progress notes on all assigned cases
  • Communicate with judges, lawyers, and DSS caseworkers regarding program participants
  • Complete and submit various reports and court updates for clients
  • Develop measurement strategies to evaluate program effectiveness

Qualifications

  • Bachelor’s degree in social or behavioral sciences required
  • Minimum of two years of relevant work experience preferred
  • Access to an automobile and valid NY State driver’s license
