As a Bilingual Advanced Practitioner, you’ll be a vital part of an integrated care team, working alongside a Registered Nurse and Social Worker. You’ll provide personalized, compassionate medical care to patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD), empowering them to improve their quality of life from the comfort of their homes and slowing the progression of their condition.

Skills

  • Fluency in English and Spanish
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certification (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Strong understanding of federal and state regulations (HIPAA and OSHA)
  • Excellent verbal and written communication skills
  • Proficiency with medical instruments and computer-based data management programs
  • Ability to work independently and autonomously

Responsibilities

  • Conduct comprehensive assessments, including annual wellness exams, in patients’ homes and via telehealth
  • Counsel and educate patients and families about available benefits and programs
  • Document patient encounters thoroughly and accurately
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Deliver evidence-based, timely care that prioritizes patient well-being
  • Prescribe medications, order tests, and collaborate with the patient’s physician
  • Improve patient quality outcomes through education and preventive monitoring
  • Actively participate in integrated care team meetings

Qualifications

  • Two or more years of direct patient care experience
  • Experience conducting annual wellness visits or similar comprehensive visits (virtually or in-home)
  • Experience with managed care/IPA/health plans

Education Requirements

  • Relevant certifications and licenses as specified in the qualifications section

Education Requirements Credential Category

  • Bilingual (English/Spanish), Active and unrestricted RN and NP or PA license, board certification, DEA certificate

Experience Requirements

  • 2+ years of direct patient care experience
