EAC Network seeks a Bilingual Nutrition Program Assistant for the Nutrition Program, located in Hempstead, NY. The position is full-time, Monday-Friday from 9:00 AM to 5:00 PM with excellent benefits including medical, dental, 401K, and very generous Paid Time Off (PTO).

Skills

  • Bilingual in English and Spanish (required)
  • Effective communication skills, both orally and in writing
  • Proficient in computer applications including Microsoft Office, spreadsheets, and database software
  • Knowledge of community services for Persons Living with HIV/AIDS in Nassau County

Responsibilities

  • Assist with coordinating nutrition health education sessions promoting retention in medical care for Spanish-speaking HIV+ clients
  • Maintain and electronically track client records and participation in program activities
  • Make and follow up on referrals for community services
  • Conduct off-site intakes and appointments for screenings
  • Collaborate with medical providers and Registered Dietitians to enhance client care
  • Document progress notes summarizing client progress on nutritional goals
  • Responsible for distributing weekly pantry bags

Qualifications

  • Associate’s degree in health or human services (required), Bachelor’s degree preferred
  • Minimum of 1 year experience in the field of HIV/AIDS or with other chronic illnesses
  • Strong interpersonal skills and ability to work cooperatively with others
