As a Case Manager Level II, you will work closely with diverse populations, including immigrant families, and collaborate with community resources to meet their unique needs. Compensation and Benefits

This full-time, non-exempt position requires 40 hours per week, typically across five days. Flexibility may be needed to meet deadlines and client needs.

Skills

  • Experience in trauma-informed care and case management.

Responsibilities

  • Manage a caseload of post-release services cases, providing ongoing support through assessments, planning, and psychoeducation.
  • Conduct home studies and comprehensive assessments to ensure a child’s safety, well-being, and proper placement.
  • Collaborate with care providers, community resources, and sponsors to verify and address needs, risks, and additional support opportunities.
  • Maintain regular contact through telephone, virtual, and in-person home visits with children and their sponsors.
  • Utilize trauma-focused approaches and brief therapeutic techniques in interactions with clients.
  • Ensure accurate and timely documentation while safeguarding confidential and sensitive information.
  • Identify and report any concerns related to children, sponsors, or significant incidents as a mandated reporter.
  • Participate in training, meetings, and quality improvement initiatives.
  • Conduct in-person home study and post-release visits, which may occasionally require evening or weekend availability.

Qualifications

  • A Bachelor’s degree in Social Work, Education, Psychology, Sociology, or a related field.
  • At least 1 year of case management experience (including writing professional assessment reports).
  • Knowledge of family preservation, kinship care, and trauma-informed practices.
