Skills

• Active multi-state/compact license (LPN) or (LVN)
  
• Strong technical aptitude, including proficiency in Microsoft Office Suite
  
• Excellent written and verbal communication skills
  
• Ability to work independently in a remote environment
  
• Ability to sit for long periods and effectively use a telephone
  
• Ability to process and document detailed information during phone conversations
  
• Bonus: Experience with Epic electronic health record (EHR) system; bilingual proficiency; additional single-state LPN/LVN licensures
  

Responsibilities

• Manage a patient census with a resolution-driven approach
  
• Identify and connect patients with community resources
  
• Provide patient education on chronic condition management
  
• Perform thorough medication management and reconciliation
  
• Assist in ensuring timely delivery of essential services to patients
  
• Address patient questions and create open lines of communication
  
• Assist with referrals and appointment scheduling
  

Qualifications

• Active multi-state/compact license (LPN) or (LVN)
  
• Strong technical aptitude, including proficiency in Microsoft Office Suite
  
• Excellent communication skills
  
• Ability to work independently in a remote environment
  

Education Requirements

• Licensed Practical/Vocational Nursing degree or equivalent
