We’re seeking a dedicated and compassionate Chronic Care Manager to join our team in a fully remote role. As a Licensed Practical Nurse (LPN), you will play a vital role in supporting the care of patients with chronic conditions, using a specialized platform designed to optimize patient care. You’ll work closely with primary care physicians and other healthcare providers to ensure patients receive the highest level of care tailored to their needs.

Skills

• Current Licensed Practical Nurse (LPN) license with 1.5 to 12 years of experience
• Strong problem-solving skills with the ability to multitask effectively
• Excellent verbal and written communication skills
• A customer-service oriented mindset, always putting patients’ needs first
• Bilingual (English/Spanish) is a plus but not required
• Experience using EHR/EMR systems to manage patient data is preferred

Responsibilities

• Work collaboratively with a care team to improve the health and well-being of chronic care patients
• Utilize the Chronic Care Management platform to coordinate care for patients managing multiple chronic conditions
• Organize and analyze patient data to identify gaps in care and unmet needs
• Assist healthcare providers in creating and implementing care plans and interventions for patients
• Manage a patient panel (approximately 500 patients, with 300 billable by the end of the month)
• Provide proactive outreach and support to patients, ensuring seamless communication between the patient, their care team, and any other necessary services
• Contribute to continuous improvement initiatives aimed at enhancing the quality and efficiency of care
• Attend team meetings and engage in professional development opportunities
• Ensure compliance with all relevant healthcare regulations and organizational policies

Qualifications

• Current Licensed Practical Nurse (LPN) license with 1.5 to 12 years of experience
• Experience working in population health or chronic care management is a plus
• Excellent verbal and written communication skills
