Responsibilities

• Work collaboratively with a care team to improve the health and well-being of chronic care patients
• Utilize the Chronic Care Management platform to coordinate care for patients managing multiple chronic conditions
• Organize and analyze patient data to identify gaps in care and unmet needs
• Assist healthcare providers in creating and implementing care plans and interventions for patients
• Manage a patient panel of approximately 500 patients, with 300 billable by the end of the month
• Provide proactive outreach and support to patients, ensuring seamless communication between the patient and their care team
• Contribute to continuous improvement initiatives aimed at enhancing the quality and efficiency of care
• Attend team meetings and engage in professional development opportunities
• Ensure compliance with all relevant healthcare regulations and organizational policies

Qualifications

• 1.5 to 12 years of experience as a Licensed Practical Nurse (LPN)
• Experience in population health or chronic care management is a plus
• Proven ability to multitask and handle various responsibilities
