Responsibilities

• Provide essential case management and supportive services to veteran participants
• Deliver mobile and field-based support focusing on independent living skills, housing stabilization, and financial management
• Utilize a strengths-based case management model to empower veterans
• Partner with clients and staff to develop personalized Housing Stability Plans (HSPs)
• Provide continuous case management support, assessing progress on HSPs
• Deliver supportive services that enhance clients’ ability to problem-solve and manage their own care
• Utilize evidence-based practices in case management

Qualifications

• Master’s Degree in a Social Service or Clinical field from an accredited program
• Registration and accreditation (or ability to obtain) with the California Board of Behavioral Sciences
• One (1) year of experience in healthcare, case management, or working with vulnerable populations

Education Requirements

• Master’s Degree in a relevant field

• Accredited social service or clinical field (e.g., MSW, MFT, Clinical Counselor, Psychology)

Experience Requirements
