We are seeking a compassionate and skilled Clinical Liaison to join our team, dedicated to coordinating exceptional clinical care for our geriatric and disabled patient community. In this pivotal role, you will be instrumental in delivering high-quality healthcare services tailored to meet the unique needs of our patients. Working collaboratively with internal and external stakeholders, you will ensure a seamless and patient-centered approach to care. This remote position requires residency in one of our designated Markets (Minnesota, Wisconsin, or Florida) and offers flexibility within our regular business hours. Responsibilities: Schedule and coordinate appointments with specialty providers to ensure timely access to care. Provide support through our clinical call line, addressing inquiries and offering necessary information to patients, families, and external healthcare agencies. Cultivate clear and efficient communication channels within the healthcare team, both internally and externally. Assist in the medication refill process and respond to pharmacy inquiries promptly. Facilitate prior authorizations for medications, specialty visits, and durable medical equipment. Support the durable medical equipment process, ensuring timely order submission and follow-up. Maintain accurate records, including entering lab results, diagnostic imaging, and immunization history. Conduct home-health reviews and provide education on healthcare directives as needed. Facilitate continuity of care by connecting patients with essential resources and services. Monitor continuous quality improvement initiatives to optimize patient outcomes. Conduct health plan delegated annual chart reviews to identify and address gaps in care. Foster positive working relationships and effective communication within and across departments to enhance patient care. Qualifications: Education/Certification/Experience: Medical certification or license required; CMA, LPN preferred. 3-5 years of relevant medical office experience. Knowledge/Skills/Abilities: Proficiency in navigating electronic medical record systems and delivering patient care. Clear and effective verbal and written communication skills. Excellent interpersonal and customer service abilities. Detail-oriented with a commitment to accuracy. Demonstrated ability to work independently while collaborating effectively within a team. Computer proficiency. Strong understanding of medical terminology and patient care documentation. Fluency in English, including reading, writing, speaking, and comprehension.
