Responsibilities

  • Conduct performance reviews and field supervision assessments for all clinical staff
  • Coordinate staff vacation schedules and ensure appropriate coverage
  • Oversee orientation and onboarding of new clinical staff in collaboration with HR
  • Collaborate with the leadership team to meet financial goals
  • Manage patient transitions from home care to hospice
  • Provide oversight for patient care services and coordinate with scheduling teams
  • Ensure patient care plans are updated and needs assessed
  • Assist with quality management and continuous improvement of home care services
  • Lead interdisciplinary team meetings and contribute to staff training and development

Qualifications

  • Associate’s degree in Nursing required; Bachelor’s degree preferred
  • Previous management experience is a plus
  • Current RN license in good standing (NH)
  • Ability to work flexible hours, including weekends if needed
  • Reliable transportation with valid driver’s license and proof of car insurance
