Responsibilities

  • Conduct surveys accrediting or certifying facilities that serve residents of long-term care facilities
  • Manage the survey process effectively and independently
  • Collaborate with internal and external stakeholders to ensure quality standards
  • Maintain accurate documentation of survey results

Qualifications

  • Must possess a Master’s degree in Social Work
  • Completion of CMS’s Long-Term Care Basic Training and passing the Surveyor Minimum Qualifications Test (SMQT)
  • Minimum of two years of recent experience as a surveyor
  • Current licensure as an LCSW
  • Proven track record of managing survey processes

  • At least two years of experience in accrediting or certifying long-term care facilities
  • Ability to travel up to 75% of the time regularly
