Skills

• Demonstrated experience in wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.

Responsibilities

• Collaborate with a Team by participating in Interdisciplinary Team (IDT) meetings
• Provide Comprehensive Assessments through face-to-face nursing assessments
• Deliver Personalized Care in partnership with medical providers
• Perform Clinical Interventions including administration of medications
• Triage and Escalate Care in the outpatient setting
• Coordinate Care Transitions with hospital and long-term care facility case managers
• Educate and Empower participants, caregivers, and team members
• Provide After-Hours Support by remotely taking after-hour calls

Qualifications

• Minimum 2 years of clinical experience caring for medically complex or older adult populations (preferred)
• Active, unrestricted RN license in California
• Graduate of an accredited School of nursing (BSN strongly preferred)
• CPR and First Aid certification (within 60 days of employment)
• Alzheimer’s certification (preferred)
• Bilingual in English and Spanish (preferred)
• Valid CA driver’s license, personal transportation, good driving record, and auto insurance

• Graduate of an accredited School of nursing
• BSN strongly preferred
