We are seeking an LPN/RN to take on the role of Director of Patient Care. In this position, you will manage a team consisting of two nurses and one social worker. This individual will collaborate to promote quality care and cost-effective outcomes, enhancing the physical, psychosocial, and vocational health of our patient population. The role is primarily remote, with occasional office work required at the Wesley Chapel office at least once per week. Join our fantastic team and enjoy an excellent schedule from Monday to Friday, 8 AM to 5 PM, with no evenings or weekends.

Responsibilities

  • Lead the patient care team, which includes two nurses and one social worker
  • Develop and administer policies and procedures for utilization monitoring and management
  • Adhere to established policies and procedures
  • Apply appropriate data collection techniques and tools
  • Maintain and improve productivity and patient care operations by monitoring team and system performance
  • Mentor and guide team members
  • Maintain good community relationships
  • Follow all federal, state, and local laws, licensure/certifications in an ethical manner
  • Collaborate and communicate effectively with other managers and departments
  • Perform additional duties as assigned

Qualifications

  • At least three years as a Director of Utilization Management, Utilization Review, Care Management, Care Coordination, or Case Management
  • Certification in the appropriate field of expertise (e.g., Certified Case Manager, Chronic Care Professional) preferred
  • Must be an LPN or RN with an active and unrestricted license to practice in Florida
  • Ability to attend to numerous details while maintaining courteous, accurate, and timely relations with a variety of individuals
  • Capacity to comprehend the consequences of various problem situations and refer such problems to the appropriate individual or supervisor for decision-making

Education Requirements

  • Must have an active LPN or RN license in Florida

Experience Requirements

  • Minimum of three years in relevant roles such as Director of Utilization Management or similar positions
