The Physician/Nurse Practitioner will conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members/patients within various settings including home visits, clinics, telemedicine, and long-term care facilities. Skills

  • 2 years of hands-on experience as a Nurse Practitioner or Physician
  • Knowledge of HCC model, HEDIS, and STAR quality measures
  • Current, unrestricted license in Connecticut or ability to acquire one
  • Registration with Medicare
  • Board certification in internal medicine, geriatric, or family medicine specialty
  • Strong communication skills with elderly and chronically ill members
  • Proficient in using telehealth technology and various digital devices

Responsibilities

  • Conduct detailed evaluations of medical and social assessments for beneficiaries
  • Identify and refer members/patients for case management as needed
  • Recommend follow-up with primary care physicians for members
  • Travel across local areas where you hold a current license
  • Document assessments within available Electronic Medical Records (EMR)
  • Complete assessments via Telehealth with audio and video support
  • Maintain current unrestricted license in Connecticut

Qualifications

  • Current unrestricted license in the state of Connecticut
  • Must live in Connecticut
  • Not required to prescribe or alter current treatment plans

Education Requirements

  • Graduate degree in nursing or medicine

Education Requirements Credential Category

  • Master’s degree or higher in a relevant field

Experience Requirements

  • Minimum of 2 years of relevant clinical experience as a Nurse Practitioner or Physician
