The Physician/Nurse Practitioner will conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace patients in various settings, including home visits, telemedicine, and clinics. Skills
- 2 years of hands-on experience as a Nurse Practitioner or Physician.
- Knowledge of HCC model, HEDIS, and STAR quality measures.
- Current, unrestricted license to practice in the state or ability to obtain one.
- Registration with Medicare and Wisconsin Medicaid.
- Board certification preferred in internal medicine, geriatric, or family medicine specialty.
- Excellent communication skills with elderly and chronically ill members.
- Proficient in using computers, tablets, and mobile technology.
- Comfortable in home visit environments or telehealth settings.
Responsibilities
- Complete detailed evaluations of the medical and social assessments of beneficiary members using Medigence Health’s tablet-based assessment tool.
- Identify and refer members/patients for routine, urgent, and emergent case management.
- Provide recommendations for patient follow-up with their primary care physician.
- Travel to local counties, regions, and/or states where you hold a current license.
- Live/work/or be willing to travel to specified counties in Wisconsin: Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, and Winnebago.
- Note that prescribing or altering members’ treatment plans is not required.
Qualifications
- 2+ years of experience as a Nurse Practitioner or Physician.
- Active, unrestricted state license or ability to obtain one.
- Registered with Medicare and Wisconsin Medicaid.
- Board certified in relevant specialties preferred.
Education Requirements
- Degree in Nursing or Medicine from an accredited institution.
Education Requirements Credential Category
- Must possess a valid Nurse Practitioner or Physician credential.
Experience Requirements
- Minimum of 2 years of direct patient care experience in a clinical setting.
