Skills

  • Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine

Responsibilities

  • Function as the day-to-day clinical leader, providing decision support to nurse practitioners
  • Perform 4-6 preventive visits daily to optimize chronic conditions and develop proactive care plans
  • Conduct urgent care visits in the home to prevent unnecessary ED transfers and hospital admissions
  • Collaborate with a multidisciplinary team including nurse care managers, behavioral health specialists, and dieticians
  • Assume responsibility as PCP where necessary to ensure continuity of care
  • Coordinate with other physicians and community-based organizations for seamless patient care transitions

Qualifications

  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution
  • Experience managing complex, high-acuity patients
  • Eagerness to work with a diverse and multidisciplinary team

  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree
