Skills

  • Board Certified / Board Eligible Physician
  • Experience in Internal Medicine, Family Medicine, or Emergency Medicine
  • Strong clinical leadership abilities
  • Ability to manage complex, polychronic patients
  • Comfort with high-acuity medical situations
  • Team-oriented mindset with a dedication to collaboration

Responsibilities

  • Function as a day-to-day clinical leader, providing decision support to nurse practitioners
  • Perform 4-6 preventive visits daily to optimize chronic conditions
  • Conduct urgent care visits in the home to avoid unnecessary ED transfers
  • Collaborate with a multidisciplinary team to meet patients’ medical and psychosocial needs
  • Assume responsibility as the primary care provider when necessary
  • Help co-manage patients with existing primary care providers
  • Coordinate with other healthcare providers to ensure smooth transitions of care
  • Offer medical direction to community organizations involved in patient care

Qualifications

  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution
  • DEA registration
  • Proven experience managing complex, high-acuity patients
  • Commitment to transforming care for the sickest and frailest populations
  • Strong collaborative skills within a multidisciplinary setting

Education Requirements

  • M.D. or D.O. from an accredited medical school

Experience Requirements

  • Experience managing complex patients in a clinical setting
  • Familiarity with coordinating care across various healthcare providers
