Responsibilities

  • Function as day-to-day clinical leader, providing decision support to nurse practitioners and directing the multidisciplinary team.
  • Perform 4-6 preventive visits daily to optimize chronic conditions and develop proactive care plans.
  • Conduct urgent care visits in the home to avoid unnecessary ED transfers and hospital admissions.
  • Leverage support from nurse care managers and other healthcare professionals to meet patients’ needs.
  • Assume responsibility as PCP when necessary and collaborate with existing PCPs for patient care.
  • Coordinate with other physicians to ensure smooth transitions and prevent readmissions.
  • Offer medical direction to community-based organizations that support patients.

Qualifications

  • Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine preferred.
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution.
  • DEA registration.
  • Experience in managing complex patients and a commitment to transforming care for marginalized populations.

Education Requirements

  • Medical degree (M.D. or D.O.) from an accredited educational institution.
