As a Physician, you will conduct comprehensive assessments compliant with the annual wellness visit, all within a home setting. Skills

  • 2 years of hands-on experience as a Physician
  • Knowledge of HCC model, HEDIS, and STAR quality measures
  • Current, unrestricted license in Wisconsin or ability to acquire one
  • Registered with Medicare and Wisconsin Medicaid
  • Board certified, preferably in internal medicine, geriatric, or family medicine specialty
  • Effective communication skills with elderly and chronically ill members
  • Comfortable working in home visit environments or using telehealth technology
  • Proficient in using computer, tablet, and mobile technology

Responsibilities

  • Complete detailed evaluations of medical and social assessments of beneficiary members using Medigence Health’s tablet-based assessment tool
  • Identify and refer members/patients for routine, urgent, and emergent case management
  • Make recommendations for member follow-up with their primary care physician
  • Travel throughout local counties, regions, and states where you hold a current license
  • Live/work/willing to travel to the following counties in Wisconsin: Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Winnebago
  • No requirement to prescribe or alter the member’s current treatment plans

Qualifications

  • Must be registered with Medicare and Wisconsin Medicaid
  • Board certified (preferably internal medicine, geriatric, or family medicine specialty)

Education Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO)

Education Requirements Credential Category

  • Medical License

Experience Requirements

  • A minimum of 2 years of experience in a clinical setting as a Physician
