The Physician will conduct comprehensive assessments compliant with annual wellness visits in a home setting. Skills

  • 2 years hands-on experience as a Physician
  • Knowledge of HCC model, HEDIS, and STAR quality measures
  • Current, unrestricted license in Wisconsin or the ability to acquire one
  • Registered with Medicare and Wisconsin Medicaid
  • Board Certified, preferably in internal medicine, geriatrics, or family medicine specialty
  • Effective communication skills with elderly and chronically ill members
  • Comfortable working in a home visit environment or using telehealth technology
  • Proficient in the use of computer, tablet, and mobile technology

Responsibilities

  • Complete a detailed evaluation of the beneficiaries’ medical and social assessments using Medigence Health’s tablet-based assessment tool
  • Identify and refer members/patients for routine, urgent, and emergent case management
  • Make recommendations for members’ follow-up with their primary care physician
  • Travel throughout local counties, regions, and/or states where you hold a current license
  • Live/work/or be willing to travel to various counties in Wisconsin, including Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, and Winnebago
  • Not required to prescribe or alter the members’ current treatment plans

Qualifications

  • Must possess a current, unrestricted license in Wisconsin or the ability to obtain one
  • Must be registered with Medicare and Wisconsin Medicaid
  • Board certification in internal medicine, geriatrics, or family medicine preferred

Education Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree

Education Requirements Credential Category

  • Medical degree from an accredited institution

Experience Requirements

  • Minimum of 2 years of hands-on experience as a Physician
