Responsibilities

• Collaborate with hospital and community-based healthcare providers for smooth patient transitions into the Hospital at Home program
• Participate in team huddles to communicate daily patient capacity and operational updates
• Coordinate patient care schedules, including diagnostics and durable medical equipment delivery
• Manage acute patient needs and resolve barriers to care with the broader care team
• Provide direct patient care in the home, ensuring high-level medical-surgical nursing services
• Utilize remote patient monitoring technology and coordinate with paramedics and ancillary services
• Monitor quality standards and contribute to continuous improvement initiatives
• Participate in case reviews to assess patient progress and adjust care plans as needed

Education Requirements

Graduate from an accredited nursing program (Bachelor’s in Nursing preferred)
