Medigence Health is dedicated to providing Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members/patients served by primary care physicians, medical groups, and health plans. The Physician/Nurse Practitioner will conduct thorough assessments that comply with annual wellness visits, in various settings including homes, clinics, via telemedicine, or in long-term facilities. Skills

  • 2 years hands-on experience as a Nurse Practitioner or Physician
  • Knowledge of HCC model, HEDIS, and STAR quality measures
  • Current, unrestricted license in the state or ability to acquire one
  • Registered with Medicare and Wisconsin Medicaid
  • Board certified, preferably in internal medicine, geriatric, or family medicine specialty
  • Effective communication skills with elderly and chronically ill members
  • Comfortable working in home visit environments or using telehealth technology
  • Proficient in the use of computer, tablet, and mobile technology

Responsibilities

  • Complete a detailed evaluation of medical and social assessments of beneficiary members using Medigence Health tablet-based assessment tool
  • Identify and refer members/patients for routine, urgent, and emergent case management
  • Make recommendations for members to follow up with their primary care physician
  • Travel throughout local counties, regions, and/or states where you hold a current license
  • Willingness to travel to the following counties in Wisconsin: Dane, Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha, Winnebago
  • Note: Not required to prescribe or alter the members’ current treatment plans

Qualifications

  • 2 years of relevant experience in healthcare as a Nurse Practitioner or Physician
  • Valid and current state license without restrictions
  • Board certification in relevant medical specialty

Education Requirements

  • Master’s degree in Nursing or Medicine
  • Current certification and registration as a Nurse Practitioner or Physician

Experience Requirements

  • Minimum of 2 years of hands-on experience in a clinical setting, preferably in primary care or geriatrics
  • Experience utilizing technology for patient assessments and communication
