Central Coast Home Health is a rapidly growing, locally owned organization with an excellent reputation in San Luis Obispo and Santa Barbara counties. We are currently seeking a full-time, part-time, or weekend-only Home Health Physical Therapist to serve the Southern San Luis Obispo areas, including Santa Maria, Orcutt, and Lompoc.

As a therapist, you will deliver high-quality care to patients in their own home environment. Collaborating with an interdisciplinary team, you will address all patient needs, develop relationships, and ensure continuity of care. This rewarding role requires strong organizational skills and the ability to work independently, managing your own caseload and schedule.

Compensation and Benefits

This position is compensated by the visit:

• Routine visits: \(67.00 - \)79.00 per visit
• Evaluations: \(90.00 - \)104.00 per visit
• Admissions: \(167.50 - \)197.50 per visit

Actual pay rates may vary based on factors including visit type, experience, location, and years of experience.

Qualifications

• Licensed Physical Therapist in the state of California
• Proven experience in home health or similar settings preferred
