Medigence Health provides Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and health plans we serve. The Physician/Nurse Practitioner will perform comprehensive assessments compliant with the annual wellness visit in various settings, including home, clinic, telemedicine, or long-term facilities. Skills

  • 2 years of hands-on experience as a Nurse Practitioner or Physician
  • Knowledge of HCC model, HEDIS, and STAR quality measures
  • Current, unrestricted license in the state or the ability to acquire one
  • Registration with Medicare and Medicaid
  • Board certification in internal medicine, geriatric, family medicine specialty, or nephrology preferred
  • Effective communication skills with elderly and chronically ill members
  • Comfortable working with telehealth technology
  • Proficient in the use of computer, tablet, and mobile technology

Responsibilities

  • Complete detailed evaluations of medical and social assessments using Medigence Health tablet-based assessment tool
  • Identify and refer members/patients for routine, urgent, and emergent case management
  • Make recommendations for members to follow up with their primary care physician
  • Travel throughout local counties, regions, and/or states where you hold a current license
  • Conduct comprehensive assessments via telehealth visit supporting audio and video connectivity
  • Not required to prescribe or alter the member’s current treatment plans

Qualifications

  • Current unrestricted license in the state of Michigan
  • Board certification, preferably in internal medicine, geriatric, family medicine specialty, or nephrology preferred

Education Requirements

  • Graduate degree in Nursing or Medicine

Education Requirements Credential Category

  • Advanced Practice Registered Nurse (APRN) or Medical Doctor (MD)

Experience Requirements

  • At least 2 years of relevant experience as a Nurse Practitioner or Physician
