Skills

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to a reliable vehicle or public transportation.
• Strong assessment and coordination skills.

Responsibilities

• Provide clinical services within your scope of practice, following state laws and the plan of care, in collaboration with the healthcare team.
• Receive referrals, assign clinicians, evaluate patient orders, and schedule start-of-care visits.
• Coordinate patient home health benefits, determine medical necessity, and manage insurance approvals.
• Continuously assess patient needs and ensure care is individualized, appropriate, and meets home health eligibility criteria.
• Review assessments and plans of care daily, providing consultations and recommendations to clinicians.

Qualifications

• Current RN licensure in the state of practice.
• Current CPR certification.
• Access to a reliable vehicle or public transportation.

Education Requirements

• Associate’s degree in Nursing (ADN) or Bachelor’s degree in Nursing (BSN).
