Description

We are seeking a compassionate and qualified Licensed Certified Social Worker (LCSW) to join our team full-time on a hybrid basis in the Tampa, FL area. The LCSW will deliver clinical care services for residents in Senior Living Communities (SLCs) in collaboration with community providers. You will establish positive, professional collaborative relationships with SLCs and primary care providers (PCPs) in program/service development and resident care coordination, providing direct clinical care to residents.

Your role will involve assisting older adults with challenges related to mental health and aging, while supporting seniors in managing their healthcare, planning for long-term care, and ensuring a good quality of life. This partially remote position allows for flexibility in work arrangements when not providing direct patient care in facilities.

Responsibilities

• Manage a panel of patients in Senior Living Communities (SLCs) with mild to moderate behavioral health conditions
• Establish and maintain positive relationships with SLC clinical and operational staff and PCPs
• Provide brief interventions using evidence-based techniques
• Monitor patients’ psychosocial needs and compliance with treatment plans
• Communicate regularly with PCPs and SLCs regarding patient progress
• Document patient progress and treatment recommendations for care coordination
• Participate in team-based care collaboration and maintain patient registry

Qualifications

• Master’s degree in social work, nursing, or applicable behavioral health disciplines
• Licensed Certified Social Worker within the state of Florida
• 3+ years of outpatient healthcare work experience, including Chronic Care Management and/or Collaborative Care Model (CoCM)

Education Requirements

• Master’s degree in social work or related field from an accredited college or university
