Skills

• Active LPN license
• Strong problem-solving skills
• Excellent written and verbal communication skills
• Strong customer service and interpersonal skills
• Experience with Electronic Health Records (EHR)/Electronic Medical Records (EMR) systems is preferred
• Bilingual in English and Spanish is a plus but not required

Responsibilities

• Support primary care providers in managing a panel of chronic care patients
• Utilize care management platforms to coordinate and track patient progress
• Identify unmet patient needs and facilitate timely interventions
• Engage patients in their own care and encourage proactive health management
• Maintain and manage patient registries to ensure accurate and organized health data
• Collaborate with healthcare teams to enhance care coordination and improve patient outcomes
• Contribute to quality improvement initiatives and population health strategies
• Provide courteous and professional patient interactions via phone and digital platforms
• Ensure compliance with all federal, state, and local healthcare policies and procedures
• Attend and participate in team meetings and training sessions as needed

Qualifications

• 1.5 to 12 years of experience (experience in population health is a plus)
