Due to rapid expansion across our clinics, Compass Health is offering a $3,000 sign-on bonus for the Nurse Care Manager position. The Nurse Care Manager will be an integral part of the person-centered integrated care team, providing direct clinical and medical services to up to 250 individuals enrolled in the Health Care Home using a whole-person approach.

Responsibilities

  • Meet with assigned clients to complete health screenings and provide education based on medical needs
  • Assist in developing treatment plans and healthcare goals for individuals with co-occurring chronic disease
  • Provide consultation and education to community and office-based staff
  • Collaborate with medical providers regarding significant health information
  • Monitor HIT tools for treatment alerts and hospital admissions/discharges
  • Review and analyze monthly care manager reports
  • Coordinate Metabolic Syndrome Screenings and follow-up as necessary
  • Perform Medication Reconciliations for clients discharged from the hospital
  • Interface with other agencies to provide holistic services
  • Travel to neighboring clinics for coverage and training as needed

Qualifications

Education Requirements

  • High School Diploma or GED
  • Associate’s or Bachelor’s degree in nursing required
  • LPN or RN licensure required

  • Familiarity with community health resources and services is a plus
