Skills

• Current and unrestricted Registered Nurse (RN) license in the state of application.
• Experience with Medicare Skilled Nursing preferred.
• Familiarity with OASIS documentation preferred.
• Strong assessment and communication skills.
• Ability to work collaboratively in an interdisciplinary team.

Responsibilities

• Provide timely and comprehensive hospice services, focusing on pain and symptom management.
• Conduct thorough assessments of patients’ physical, emotional, and spiritual needs.
• Coordinate care with interdisciplinary team members and attending physicians.
• Develop and implement individualized care plans.
• Ensure continuity of care during transitions between settings.
• Effectively communicate with patients, families, and care providers.
• Educate and support patients and their families in the hospice care process.
• Supervise and mentor Hospice Aides and Licensed Practical Nurses.
• Facilitate hospice services for patients in long-term care facilities.
• Participate in on-call rotation to provide after-hours care.
• Monitor patient eligibility for hospice services and document accordingly.

Qualifications

• Minimum of one year of RN experience in a hospice setting preferred.
• Strong documentation and assessment skills.

Education Requirements

• Current RN license in the state of application.
