A growing, physician-led managed care organization in Deltona, Florida is seeking a Board Certified or Board Eligible Internal Medicine or Family Medicine physician for a full-time outpatient primary care position. This role is a 100% outpatient, value-based care position focused on adult and geriatric Medicare Advantage patients — Monday through Friday, with no overnight call, weekends, or holidays.

Responsibilities

• Provide comprehensive outpatient primary care to adult and geriatric Medicare Advantage patients
• Manage chronic conditions including hypertension, diabetes, COPD, and cardiovascular disease
• Conduct Annual Wellness Visits
• Apply HEDIS quality measures within a value-based care model
• Develop individualized care plans with multidisciplinary team support
• Maintain accurate and timely clinical documentation

Qualifications

• MD or DO from an accredited U.S. medical school
• Active Florida medical license or eligibility to obtain
• Managed care experience preferred but not required
