We are looking for an experienced Physician Assistant or Nurse Practitioner to join our practice part-time in Fort Wayne, IN. The ideal candidate will be a compassionate provider who will manage inpatients at our facility. The patient diagnostic complexity includes managing trach/vent, complicated wounds, and multiple co-morbidities. You must be comfortable managing acute changes in place and will collaborate with corporate leadership to assist in maintaining optimal momentum of our stellar quality metrics, program components, and implementing proprietary protocols to achieve clinical outcomes.

Skills

  • Minimum 3 to 5 years of acute inpatient hospital experience as a Physician Assistant or Nurse Practitioner is required
  • Expertise in primary care, acute care, and chronic care practices
  • Valid Nurse Practitioner or Physician Assistant license in IN
  • Master’s degree (graduation from an accredited NP or PA program)
  • ACLS Certified

Responsibilities

  • Perform history and physical exams on patients
  • Order and study diagnostic tests (blood tests, X-rays, etc.)
  • Diagnose, manage, and treat acute episodic illnesses and stable chronic conditions
  • Prescribe medications and write referrals
  • Maintain accurate records and schedules for patient progress
  • Inform and educate patients about health conditions and prescribed medications
  • Collaborate with the healthcare team to monitor and evaluate clinical problems
  • Identify patient needs and promote good health practices
  • Advance professional knowledge and conduct research
  • Follow up with discharged patients to assess ongoing needs
