Skills

• Graduate of an accredited School of Nursing.
• Current RN license in the State of Michigan.
• Minimum of one year of experience as a Registered Nurse, with at least one year in Home Health Care.
• Strong knowledge of primary care practices and regulations.
• Excellent interpersonal and communication skills.
• Ability to perform physical tasks, including lifting and standing, as needed.
• Reliable transportation and the ability to travel within the service area.
• Ability to work in a hybrid environment.
• Residency within the state of Michigan.

Responsibilities

• Patient Advocacy: Champion your patients’ needs, ensuring they receive appropriate services based on their choices and program eligibility.
• Care Planning: Develop comprehensive care plans based on thorough patient assessments.
• Communication & Collaboration: Effectively communicate with patients, families, medical professionals, and our internal team, both verbally and in writing.
• Documentation: Maintain accurate, complete, and timely patient documentation.
• Emotional Support: Provide social and emotional support, offering problem-solving techniques to patients and their caregivers.
• Coordination & Monitoring: Ensure seamless coordination of care and monitor patient progress.

Qualifications

• Minimum of one year of experience as a Registered Nurse.
• At least one year in Home Health Care.
• Case Management certification is a plus!
• Excellent interpersonal and communication skills.
