The Speech Therapist (Pathologist) will provide speech therapy services to Agency patients as prescribed by the attending physician. This role includes assessing, planning, and implementing rehabilitation services for speech, language, and/or swallowing disorders. You will also evaluate speech and language abilities through diagnostic testing and report progress to the physician, while instructing family members and healthcare team members on methods to assist patients in improving communication skills.

Skills

  • Valid SLP certification/license in the state of Illinois (IL)
  • Experience in Home Health Care or related fields
  • Strong assessment and diagnostic skills
  • Ability to communicate effectively with patients and families
  • Knowledge of rehabilitation programs for speech and language disorders
  • Capable of maintaining clinical documentation and reports

Responsibilities

  • Assess, plan, and implement rehabilitation services for speech, language, and/or swallowing disorders.
  • Provide evaluation of speech and language abilities through diagnostic testing.
  • Record and report the client’s progress in treatment to the physician.
  • Instruct family members and healthcare team members in assisting the patient in improving communication skills.
  • Re-evaluate patient’s progress and established rehabilitation program as necessary.
  • Prepare periodic reports for the Agency, sent to the physician and third-party payers.
  • Make appropriate referrals to outside audiology speech clinics or community services as necessary.
  • Maintain clinical and progress notes, submitting paperwork on time according to Agency policy.
  • Provide liaison contact and follow-up with community agencies.
  • Participate in in-service, case conferences, and staff meetings as required.

Qualifications

  • Master’s degree or higher in Speech-Language Pathology
  • Supervised Post-Certification in an institutional setting (1+ years)
  • 1+ years of experience in Home Health Care or similar settings

  • Master’s degree in Speech-Language Pathology or related field
