Responsibilities

• Provide specialized PM&R services as a rehab consultant in a skilled nursing facility
• Collaborate with the care team to manage conditions such as CVA, progressive neurological disorders, and postoperative orthopedic recovery
• Work closely with physical, occupational, and speech therapists to improve patient function and quality of life
• Assess and manage therapy barriers, including pain, spasticity, and neurogenic conditions
• Participate in multidisciplinary team meetings to bridge therapy and internal medicine practices
• Engage in continuous learning through monthly didactic sessions and team collaboration events

Qualifications

• Proven experience in skilled nursing or post-acute care environments (preferred)
• Demonstrated success in a collaborative healthcare environment
