As a Landmark Physician, you will serve a dual role as a practicing physician providing direct patient care (60%) and as a Pod Leader overseeing the clinical team (40%). This unique position allows you to gain valuable leadership experience while actively contributing to patient health and wellness. You will visit your panel of patients in their homes, spending the necessary time to address their health issues and fostering trusting relationships with them and their families. By observing patients in their own environments, you will gain insights that may not be revealed in traditional care settings.

In this role, you will direct a collaborative, multidisciplinary clinical team working alongside you. You will have access to our electronic medical records (EMR) program, mobile medical technology system, and dedicated technical support.

Skills

  • Board certification (or eligibility) in Internal Medicine, Family Medicine, or Emergency Medicine
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution
  • Experience managing complex, poly-chronic patients
  • Comfort in working with high-acuity and geriatric patients
  • Strong communication and teamwork skills
  • Reliable transportation for patient visits, complying with Landmark’s Motor Vehicle Safety policy

Responsibilities

  • Act as the clinical leader, providing support to Advanced Practice Providers (APPs) and directing the multidisciplinary team
  • Conduct 4-6 preventive visits daily to manage chronic conditions and develop care plans
  • Perform urgent care visits in patients’ homes to minimize emergency department transfers
  • Collaborate with Nurse Care Managers, Behavioral Health specialists, and other professionals to address patients’ medical and biopsychosocial needs
  • Assume responsibility as a primary care provider when necessary and coordinate with existing PCPs
  • Facilitate transitions of care across various healthcare settings to prevent readmissions
  • Offer medical direction to community-based organizations impacting patient lives

Qualifications

  • Must be board certified at the time of hire or obtain certification within five years post-residency
  • Experience managing complex patients, particularly in geriatric care
  • Dedication to improving care for marginalized populations
  • Willingness to work with a diverse, multidisciplinary team

Education Requirements

  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited educational institution

Education Requirements Credential Category

  • Medical Degree

Experience Requirements

  • Relevant experience in managing complex, polychronic patients
  • Experience working with geriatric populations is preferred
