As a Landmark Physician, you will serve in a dual capacity as a practicing Physician providing care to patients (60%) and as a Pod Leader offering oversight and guidance for clinical team members (40%). This unique role enables you to gain valuable leadership experience while actively contributing to patients’ health and wellness.

You will visit your panel of patients in their homes, ensuring that their health issues are addressed thoroughly. By fostering friendly and trusting relationships, you will gain insights into their lives that may not be revealed in traditional care settings. As a Physician (Pod Leader), you will lead a collaborative, multidisciplinary clinical team, supported by our custom electronic medical records (EMR) program and comprehensive mobile medical technology system.

Responsibilities

  • Provide clinical leadership and decision support to Advanced Practice Providers (APPs)
  • Conduct 4-6 preventive visits daily, optimizing chronic conditions and developing proactive care plans
  • Perform urgent care visits in patients’ homes to avoid unnecessary hospitalizations
  • Collaborate with Nurse Care Managers, Behavioral Health specialists, Social Workers, and more to meet patients’ needs
  • Assume responsibility as the primary care provider when necessary
  • Co-manage patients with existing primary care providers
  • Coordinate with other healthcare providers to ensure smooth transitions of care

Qualifications

  • Must be board certified at the time of hire or obtain certification within five years post-residency
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution
  • DEA registration
  • Experience managing complex, polychronic patients
  • Eagerness to work with a diverse, multidisciplinary team
