Skills

  • Proficient in outpatient medicine, continuity of care, health maintenance, and disease prevention
  • Autonomy and proficiency in comprehensive health assessments
  • Strong communication skills with all members of the care team
  • Knowledge of the Medicare Advantage population and managing multiple comorbidities
  • Experience with Electronic Medical Records (EMR), specifically ECW
  • Knowledge of HEIDS, MRA, and ICD-10 Coding is a plus

Responsibilities

  • Lead a team-based medical approach to patient care
  • Focus on health maintenance, disease prevention, and continuity of care
  • Conduct comprehensive health assessments and communicate care plans effectively
  • Manage and treat the Medicare Advantage population with multiple comorbidities
  • Facilitate an effective plan of care to avoid unnecessary duplication of testing and fragmentation of care
  • Adhere to the scope of practice in your state of licensing

Qualifications

  • Graduate from an accredited MD or DO program
  • Specialization in Family Medicine, Internal Medicine, or Geriatric Medicine
  • Unrestricted medical license and DEA licensure in the state of practice
  • Up-to-date CPR certification and Basic Cardiac Life Support certification
  • Strong organizational and prioritization skills

Education Requirements

  • Graduate from an accredited MD or DO program

Education Requirements Credential Category

  • Medical Doctor (MD) or Doctor of Osteopathy (DO)

Experience Requirements

  • Experience in outpatient care and managing patients with complex health needs
  • Demonstrated success in a primary care setting with a focus on quality patient care
