Skills

• Active LPN License in Massachusetts (or a Compact License with the ability to obtain MA licensure).
• 5+ years of patient-centered care experience with strong clinical knowledge.
• Tech-savvy and comfortable working with EMR systems and digital platforms.
• Excellent verbal communication skills and a passion for patient engagement.
• Self-motivated and organized with the ability to work independently.
• Bilingual (Spanish-English) preferred.

Responsibilities

• Conduct monthly check-ins with patients, offering education and connecting them to necessary resources.
• Develop and maintain individualized care plans for timely and appropriate care.
• Track patient health status, provide proactive interventions, and prevent gaps in care.
• Assess EMRs to determine eligibility for care management services.
• Follow Medicare, HIPAA, and healthcare policies while maintaining accurate documentation.
• Meet daily performance targets, ensuring each patient receives a minimum of 20 minutes of care coordination per month.

Qualifications

• Strong clinical knowledge and experience in patient-centered care.

• LPN license from an accredited program.
