Skills

• Current Registered Nurse (RN) license in the state of practice
• CPR certification (required)
• Valid driver’s license, reliable transportation, and proof of insurance

Responsibilities

• Oversee and coordinate clinical services for home health patients
• Provide nursing care as prescribed by the physician, following all applicable regulations and policies
• Review and assign referrals, ensuring patients receive timely and appropriate care
• Assess home health benefits, medical necessity, and insurance approvals
• Continuously evaluate patient needs and ensure care plans are individualized, appropriate, and in line with physician orders
• Review daily assessments and care plans, collaborating with clinicians to ensure the best possible patient outcomes

Qualifications

• Proven experience in home health nursing
• Ability to lead and support a patient-facing team
• Excellent communication and organizational skills

Education Requirements

• Bachelor’s degree in Nursing or a related field preferred

Education Requirements Credential Category

• Registered Nurse (RN)

Experience Requirements

• Previous experience in a clinical management or leadership role is preferred
• Home health experience is a plus
