We are seeking an experienced and compassionate social worker to join a dedicated team focused on improving the quality of life for senior adults and individuals with disabilities. As a member of the Home and Community-Based Services (HCBS) team within the Department of Aging and Disability Services, you will play a key role in enhancing the independence and well-being of older adults and people with disabilities in the community.

Responsibilities

  • Client caseload management, ensuring timely and effective support for older adults and their caregivers
  • Conduct home visits to assess eligibility for various programs and services
  • Maintain accurate documentation while adhering to professional standards and regulatory requirements
  • Stay informed about aging and disability issues and available resources
  • Collaborate with other Aging and Disability programs for knowledge-sharing
  • Assist with other projects and tasks as directed by the program manager

Qualifications
The ideal candidate will have:

  • A Bachelor’s degree in Social Work or a related field
  • Experience in geriatrics and case management
  • Strong communication skills
