Description

The Field Registered Nurse will manage all aspects of patient care related to services provided in the home. This includes conducting home visits, completing all required documentation, communicating with the Primary Care Physician (PCP), updating medication profiles, coordinating care with other disciplines, and participating in case conferencing with the respective supervisor.

• Salary range: \(95,000 - \)110,000 / year
• 4 weeks of vacation
• 5 personal days
• 8 sick days per year

Skills

Responsibilities

• Perform home care assessments to determine patient eligibility for services.
• Develop and implement a Plan of Care in conjunction with the patient’s family and physician.
• Provide skilled nursing care as outlined in the Plan of Treatment.
• Supervise and orient home health aide personnel.
• Evaluate the effectiveness of nursing interventions.
• Document all assessments and services provided in clinical software.
• Manage a caseload including ongoing communication with the MD and interdisciplinary team.
• Conduct discharge planning and communicate anticipated discharge dates.
• Participate in quality improvement activities.

Qualifications

• Must have at least 1 year of hospital experience.
• Active New York state nursing license.
• Experience in a home care setting is a plus.

Education Requirements

• Associate’s Degree in Nursing or higher is preferred.
